If you do not want your information shared or permission withdrawn you can do so at any time.
How long we retain your data
Personal data, including medical records, for the purposes of private medical care are held for 7 years and then destroyed. For people aged 16 years this data will be held until age 25 years then destroyed.
All personal data eg from completed forms is held behind password protected administration systems with two factor authentication and industry standard encryption.
What rights you have over your data
You can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
